If you would like to become a member of Matthew’s Hub please:

• Complete our online form
• Download a copy of our application form
• Return it to us at hello@matthewshub.org with evidence of your diagnosis or that you are on the waiting list for assessment. 
• Alternatively you can call us on 01482 221018 to discuss your application.
